LIKENESS, PHOTO, VIDEO AND SOUND RECORDING RELEASE AND CONSENT FORM

I, hereby irrevocably grant and give permission to Ozone Music Education Foundation, Inc. (“Foundation”) and its board members, directors, officers, committee members, agents, representatives, volunteers, designees, successors, licensees and assigns, the irrevocable right and license, but not the obligation, with or without my knowledge, to film, tape, photograph and make video and sound recordings of me (collectively, "Recordings") at, during and/or in connection with the Ozone Songwriter Festival and any activities and/or events related to, incidental to, in association with, in conjunction with and/or a part of the Festival, including, without limitation, the Ozone Songwriter Night and/or Symposium/Workshop and Open Mic (collectively, the “Festival”); to broadcast, exhibit, reproduce, transmit and to show all or part of such Recordings, live or by replaying Recordings, on-line and on the Foundation's website and/or on its social media pages, including, without limitation, Facebook, Instagram and YouTube; and, to otherwise use my appearance, name, image, likeness, voice, singing voice, songs, conversation, actions, sounds, application information, biographical data, and any photographs, audio recordings, or, other materials provided by me in connection with the Festival, in any manner in the Foundation’s sole election and discretion, including, without limitation, in its publications, in newspapers, magazines and other print media, on television, radio and electronic media (including the Internet) and/or in mailings for advertisement and awareness, which use shall not entitle me to receive any compensation or other consideration whatsoever.

I hereby release, discharge and hold harmless the Foundation and its board members, directors, officers, committee members, agents, representatives, volunteers, designees, successors, licensees and assigns (collectively, the “Released Parties”) from any and all liability or claims arising out of or in any way resulting from my participation in the Festival, any Recordings of me taken or made by, for and/or on behalf of the Released Parties at, during and/or in connection with the Festival, any use or reuse of such Recordings by the Released Parties and any use or reuse by the Released Parties of my appearance, name, image, likeness, voice, singing voice, songs, conversation, actions, sounds, application information and biographical data and I agree to not make any claim against the Released Parties as a result of my participation in the Festival, any Recordings of me taken or made by, for and/or on behalf of the Released Parties at, during and/or in connection with the Festival, any use or reuse of such Recordings by the Released Parties and any use or reuse by the Released Parties of my appearance, name, image, likeness, voice, singing voice, songs, conversation, actions, sounds, application information and biographical data (including, without limitation, any claim based upon defamation, libel and/or invasion of privacy).

My release and consent herein is given voluntarily and in perpetuity. I acknowledge that I am fully aware of the contents of this Release and that I am under no disability, duress, or undue influence at the time of my signing of this Release.
